Deleting your data does not by itself disconnect the app from your Facebook account. To revoke its access, open Settings & Privacy → Settings → Apps and Websites on Facebook, select the app, and choose Remove. Doing both revokes future access and clears what was already received.
Anything unclear about a request, or about what is held, can go to info@jswebdevs.com. What is collected in the first place is set out in the Privacy Policy.
A deletion request does not need a form — an email with your name and address is enough.
